Sildenafil is the generic active ingredient used in Viagra® and other prescription products. Viagra is a registered brand name of its manufacturer. NORBU MD uses generic terminology unless referring specifically to a branded product, and does not represent that branded Viagra is what will be prescribed. Products containing the same active ingredient can differ in formulation, labeling and approved use.
On-demand treatment
Sildenafil is commonly prescribed for use in relation to anticipated sexual activity rather than as a continuous daily ED treatment. Timing and individual response can vary based on the person, the prescribed product, food, alcohol, other medications and overall health — which is why instructions come from your physician rather than a general guide.
Sildenafil has a long history of prescription use for erectile dysfunction and is well characterized in the medical literature.
It is generally used around anticipated sexual activity rather than taken continuously as a daily ED treatment.
Your physician determines whether sildenafil, the prescribed dose and the overall treatment approach are appropriate for you.
The basics
Sildenafil is a prescription medication in the class known as PDE5 inhibitors. It has a long history of prescription use for erectile dysfunction and remains one of the most widely prescribed ED treatments in the United States.
During sexual stimulation, the body signals blood vessels in penile tissue to relax so blood flow can increase. Sildenafil supports that pathway — it helps the process your body already uses to achieve and maintain an erection.
Sildenafil requires a prescription. Whether it is appropriate for you, and how it would be prescribed, is a medical decision your NORBU MD physician makes after reviewing your health history, medications and clinical factors.
Sildenafil does not automatically cause an erection. Sexual stimulation is generally still required for the medication to support an erectile response.
Sildenafil does not increase sexual desire and does not directly increase testosterone. Low libido and hormonal concerns are separate clinical questions your physician can evaluate.
Erectile dysfunction can involve vascular, metabolic, hormonal, neurological, medication-related and psychological factors. Sildenafil does not treat every underlying cause, which is one reason evaluation comes before treatment.
Timing
Sildenafil is typically prescribed for use before anticipated sexual activity. The concepts below are general patient education, not instructions — your actual timing comes from your prescription and your physician.
NORBU MD does not publish a dosing schedule, recommend a strength or tell you when to take sildenafil. Follow the prescription and instructions you receive, and talk to your physician before changing anything about how you use your medication.

Sexual stimulation triggers nerve and chemical signaling that tells blood vessels in penile tissue to relax. Without that signal, there is nothing for the medication to support.
An enzyme called PDE5 degrades part of that signaling. When PDE5 activity dominates, blood flow may not increase enough to achieve or maintain an erection.
Sildenafil inhibits PDE5 so the natural signaling can persist, supporting blood flow to penile tissue and the erectile response during sexual stimulation.
This is a simplified explanation of a physiological pathway. Individual response varies, and erectile function also depends on circulation, nerves, hormones, medications and overall health.

Compare ED treatments
Both are established prescription options for erectile dysfunction, but they differ in ways that may make one more appropriate for a particular patient — including how they're generally used, their relative duration of action, food and timing considerations, and the fact that both require a prescription and a physician evaluation. Neither option is automatically better.
Other options
Some patients don't get the response they're looking for from sildenafil or another single medication. That's a reason to talk with your physician about options — not a reason to change your medication on your own.
In certain circumstances, a physician may consider a combination treatment approach when clinically appropriate. Combination treatment is not automatically a next step after sildenafil, is not represented as stronger or superior, and is not right for everyone. Whether it's appropriate is strictly a physician decision made after evaluation.
Explore combination treatment →Is it right for you?
How often you need treatment, your timing preferences, past experience with ED medications and your overall health can all point toward different options. The only way to know which treatment — if any — is appropriate is through an individual medical evaluation. Your physician will review these factors and determine which treatment options may be appropriate.

The bigger picture
Erectile dysfunction can be a standalone issue, but it can also be connected to broader health. That's one reason a physician evaluation, rather than a self-selected medication, is the starting point for treatment.
Cardiovascular health
Blood flow is central to an erection, so cardiovascular health and circulation can play a role in ED.
Blood pressure
Both high and low blood pressure, and some blood-pressure medications, can be relevant to ED and to treatment safety.
Metabolic health
Conditions such as diabetes can affect the nerves and blood vessels involved in erectile function.
Hormonal factors
Hormone levels can contribute to ED for some patients and are a separate clinical question your physician can evaluate.
Medications
Certain medications prescribed for other conditions can contribute to ED as a side effect.
Psychological factors
Stress, anxiety, depression and relationship factors can all play a role in erectile function.
Lifestyle
Sleep, alcohol use, smoking and physical activity can all influence erectile function.
Other conditions
ED can sometimes be an early sign of another underlying health issue, which is one reason a medical evaluation matters.
Safety
Sildenafil is a prescription medication with real warnings, precautions and possible interactions. Your physician reviews these with you, and you should read the full prescribing information for the specific product prescribed to you.
Nitrates: sildenafil can cause a dangerous drop in blood pressure when combined with nitrate medications such as nitroglycerin or isosorbide. If you take nitrates in any form, tell your physician — sildenafil should not be used when it is contraindicated. Cardiovascular considerations matter as well: sexual activity carries cardiac risk for some patients, and certain heart conditions, recent cardiac events, or low or unstable blood pressure may make treatment inappropriate. Alpha-blockers, some blood-pressure medications, and certain antifungals, antibiotics and antiretrovirals can interact. Priapism — an erection lasting more than four hours — requires emergency care, as delay can cause permanent damage. Seek urgent medical attention for sudden vision loss or sudden hearing loss or ringing in the ears, chest pain during sexual activity, fainting, or signs of an allergic reaction. Alcohol can add to blood-pressure effects. This summary is not complete safety information.
Every prescription medication has risks, warnings, contraindications and possible interactions. Review the full prescribing information for any product prescribed to you, and tell your NORBU MD physician about all medications, supplements and health conditions before starting or changing treatment. Seek medical care promptly for any symptom that concerns you.
The NORBU standard

Questions
Sildenafil is a prescription medication in the PDE5 inhibitor class. It is the generic active ingredient used in Viagra® and other prescription products, and requires evaluation by a licensed physician.
Sildenafil is used for erectile dysfunction, and certain products containing sildenafil have other FDA-approved indications. Which use, if any, applies to you is a clinical decision made by your physician.
During sexual stimulation, the body signals blood vessels in penile tissue to relax so blood flow can increase. Sildenafil inhibits the PDE5 enzyme that breaks down part of that signaling, supporting your body's natural erectile response.
Onset varies between individuals and can be influenced by the product prescribed, food, alcohol and other factors. Follow the instructions provided with your prescription rather than a general timeline you read online.
Sildenafil generally has a shorter duration of action than tadalafil, which is why it is typically used in relation to anticipated sexual activity. Duration does not mean a continuous erection, and individual response varies.
No. Sildenafil does not create an erection on its own. It supports the physiological pathway involved in an erection when sexual stimulation occurs.
Generally yes. Sexual stimulation is what starts the signaling that sildenafil supports.
Sildenafil is the active ingredient; Viagra® is a registered brand name for a product containing it. Products with the same active ingredient can differ in formulation, labeling and approved use, so your physician and pharmacy identify exactly what is prescribed.
Both are PDE5 inhibitors. Sildenafil is shorter-acting and generally used around anticipated sexual activity, while tadalafil has a longer duration of action and may be prescribed as-needed or with certain regular dosing approaches depending on the patient and indication.
Neither is automatically better. The appropriate choice depends on your health, other medications, previous response, side effects, goals and preferences — which is why it is a clinical decision rather than a comparison-chart decision.
Sildenafil for ED is generally prescribed in relation to anticipated sexual activity rather than as a continuous daily treatment. How and how often to use your medication comes from your prescription and your physician.
Food, particularly a heavy or high-fat meal, can affect how some patients experience sildenafil. Your physician and the prescribing information for your product can explain what applies to you.
Alcohol can add to the blood-pressure effects of sildenafil and may contribute to symptoms such as dizziness or flushing. Discuss alcohol use with your physician.
No. Sildenafil does not directly increase testosterone. If low testosterone is a concern, that is a separate clinical evaluation.
No. Sildenafil is not an aphrodisiac and does not increase libido. It supports the physical erectile response during stimulation.
Commonly reported effects may include headache, flushing, indigestion, nasal congestion, dizziness and visual changes such as color-tinted vision. Serious risks also exist — review the full prescribing information for your product and discuss any symptoms with your physician.
Sildenafil is not appropriate for everyone. Nitrate use, certain cardiovascular conditions, low or unstable blood pressure, some eye and organ conditions, and certain medications can make treatment inappropriate or require caution. Your physician screens for this during your evaluation.
Sildenafil can cause a dangerous drop in blood pressure when combined with nitrates such as nitroglycerin or isosorbide. If you take nitrates in any form, tell your physician — sildenafil should not be used when it is contraindicated.
Yes. Alpha-blockers and certain other blood-pressure medications can interact with sildenafil and affect blood pressure. Give your physician a complete list of everything you take, including supplements.
An erection lasting more than four hours requires urgent medical attention. Seek emergency care — delaying treatment can cause lasting damage.
Combining ED medications is not something to do on your own. In certain circumstances a physician may consider customized treatment approaches, including compounded formulations, when clinically appropriate. That is strictly a physician decision, and compounded products are not FDA-approved.
Yes. Sildenafil is a prescription medication in the United States and requires evaluation by a licensed physician.
The physician visit is $29. It covers your private online visit with a licensed NORBU MD physician.
No. The $29 fee covers the physician visit only. Medication, laboratory testing and other services, if applicable, are separate.
Yes. Your visit takes place online with a board-certified physician, and your medical information is handled confidentially as part of your care.
Yes. Care is ongoing. Your physician can adjust the approach, switch treatments or stop treatment based on your response, side effects and medical need.
