Same goal. Different approach.
Tadalafil and sildenafil are both prescription PDE5 inhibitors used to treat erectile dysfunction, but differences in timing and duration can make one approach more suitable than another for certain patients. Learn the differences, then meet privately with a NORBU MD physician to discuss which option may be appropriate for you.

The short answer
Known for a longer duration of action, which may provide greater flexibility around timing.
Generally used closer to anticipated sexual activity and has a shorter duration of action.
Tadalafil means more flexibility around timing. Sildenafil means more on-demand planning. But neither is automatically better — your health, medications, preferences and treatment response all matter.
The better question isn't “Which pill wins?”
It's “Which approach fits my health and my life?”

How they work
PDE5 inhibition
Tadalafil is a PDE5 inhibitor. PDE5 is an enzyme that breaks down a signaling molecule involved in relaxing smooth muscle and allowing blood flow into penile tissue during sexual arousal.
By acting on that enzyme, tadalafil supports the body's own response. Its longer duration of pharmacologic effect is what distinguishes it most from sildenafil.
PDE5 inhibition
Sildenafil acts on the same enzyme and the same general pathway. The mechanism is not fundamentally different from tadalafil's.
What differs is pharmacokinetics — how quickly the medication is absorbed and how long its effect persists — which is why it is generally used in relation to anticipated sexual activity.
Sexual stimulation
Natural signaling
PDE5 inhibition supports the response
Increased blood flow
Erectile response
Because the pathway is shared, the meaningful differences between these medications are about timing, duration, tolerability and how treatment fits your life — not about one being a fundamentally more powerful drug.
Timing
Both medications begin working within a period described in their FDA-approved prescribing information, and for many patients that difference is smaller than expected. Sildenafil's absorption can be slowed by a high-fat meal, while tadalafil's absorption is not considered to be meaningfully affected by food — so in practice, circumstances often matter as much as the molecule. Published onset ranges describe what was observed in studied populations. They are not guarantees, and your physician can explain what to expect for anything prescribed to you.
The fastest medication isn't necessarily the best treatment for a particular patient.
Duration
Tadalafil generally has a longer duration of pharmacologic effect than sildenafil. That is the clearest, most established difference between the two medications — and it is the reason most patients end up preferring one approach over the other.
A longer treatment window may provide more flexibility around when sexual activity occurs, so treatment is less tied to a specific planned moment.
Sildenafil's shorter window is why it is generally used in relation to anticipated sexual activity. For patients comfortable with that rhythm, it is straightforward.
Duration does not mean an erection lasting for the medication's entire treatment window. An erection is not expected to persist for hours, and sexual stimulation is generally required for an erectile response. An erection lasting more than four hours (priapism) is a medical emergency and requires immediate care.
Longer isn't better. Longer is just different — and whether that difference matters depends on you.
Lifestyle
This is often the question that actually decides the conversation. Not potency — rhythm.
Tadalafil may be prescribed using different approaches, including certain regular or as-needed regimens depending on the patient and the indication your physician is treating.
Potential lifestyle consideration
With certain prescribed approaches, there is less emphasis on timing medication immediately around anticipated sexual activity.
Sildenafil is generally used as an on-demand treatment for erectile dysfunction, taken in relation to anticipated sexual activity.
Potential lifestyle consideration
A straightforward approach for patients comfortable planning medication use around sexual activity.
This is not dosing guidance and not a recommendation of either approach. How and when to use any medication is set by your physician and described in the approved prescribing information for the specific product prescribed to you.
Discuss my preferences →Tolerability
Because both medications act on the same pathway, the commonly reported effects overlap substantially. Frequency and severity vary between individuals, and neither medication is categorically better tolerated than the other.
Back pain and muscle aches are described among the effects that may occur with tadalafil.
Visual disturbances, such as changes in color perception or blurred vision, are described among the effects that can occur with sildenafil.
These are not the only possible adverse effects. Individual tolerability varies, and the full list of adverse reactions appears in the FDA-approved prescribing information for any product prescribed to you. Report any side effect to your physician rather than changing treatment on your own.
Nitrates
Tadalafil and sildenafil can cause dangerous drops in blood pressure when used with nitrate medications such as nitroglycerin or isosorbide, including recreational nitrates known as poppers. Patients using nitrates must disclose this to their physician and should not use PDE5 inhibitors when contraindicated.
Both medications carry important warnings, precautions and contraindications. Serious risks include significant drops in blood pressure, fainting, priapism (an erection lasting more than four hours, which requires emergency care) and sudden vision or hearing loss. Seek urgent medical attention for chest pain, fainting, a prolonged erection, or sudden changes in vision or hearing. Tell your physician about your cardiovascular history, blood pressure, eye and hearing conditions, liver and kidney health, and every medication and supplement you take — including nitrates, alpha-blockers, other blood-pressure medications, certain antifungals and antiretrovirals.
Every prescription medication has risks, warnings, contraindications and possible interactions. Review the full prescribing information for any product prescribed to you, and tell your NORBU MD physician about all medications, supplements and health conditions before starting or changing treatment. Seek medical care promptly for any symptom that concerns you.
The real question
Neither medication is universally better. Both are established prescription PDE5 inhibitors, and the more useful question is which approach fits your medical situation and your life. That is a decision a physician makes with you, after reviewing your health.
Don't choose based on which pill wins an internet comparison. Choose based on what makes sense for your health and your life.
More options
If your current treatment isn't giving you the experience you expected, that doesn't automatically mean you need “stronger” medication. Very often something else explains it — and that is worth a conversation before anything changes.
Timing, food, alcohol and expectations all affect experience, and small adjustments sometimes matter more than a different molecule.
A physician can review whether the approach you were prescribed still fits your situation.
Tolerability and interactions can shape which options remain appropriate for you.
ED can involve vascular, metabolic, hormonal, neurological and psychological contributors that deserve attention in their own right.
In selected circumstances, a physician may consider customized treatment options, including a compounded tadalafil + sildenafil formulation, when clinically appropriate.
Sometimes the right next step is a different single medication, or addressing a contributing health factor first.
Compounded medications are not FDA-approved, and the FDA does not review them for safety, effectiveness or manufacturing quality before marketing in the way it reviews FDA-approved drugs. Combination treatment is not appropriate for everyone, is not an upgrade tier, and requires physician evaluation.

Both are prescription PDE5 inhibitors used to treat erectile dysfunction and work through the same general pathway. The main practical differences are duration and timing: tadalafil has a longer duration of pharmacologic effect, while sildenafil has a shorter one and is generally used in relation to anticipated sexual activity.
Tadalafil is the active ingredient in Cialis®, which is a registered brand name of its manufacturer. Tadalafil is also available in generic prescription products containing the same active ingredient.
Sildenafil is the active ingredient in Viagra®, a registered brand name of its manufacturer. Sildenafil is also available in generic prescription products containing the same active ingredient.
Both begin working within the timeframes described in their approved prescribing information, and the difference is often smaller than people expect. Onset varies between individuals and can be affected by dose, food, alcohol, other medications and physiology. A high-fat meal can slow sildenafil's absorption; tadalafil's absorption is not considered to be meaningfully affected by food.
Tadalafil generally has a longer duration of pharmacologic effect. That describes a window during which the medication may help support an erectile response when sexual stimulation occurs — not a continuous erection.
Neither is universally better. They are different approaches, and which may be appropriate depends on your medical history, current medications, previous treatment, tolerability and preferences.
No. Neither medication is categorically stronger. They differ in duration and timing characteristics rather than in being more or less powerful, and doses are not directly comparable between them.
Neither has categorically fewer. Commonly reported effects overlap and include headache, flushing, indigestion, nasal congestion and dizziness. Back pain and muscle aches are described with tadalafil; visual disturbances are described with sildenafil. Individual tolerability varies.
Tadalafil has approved dosing approaches that can include once-daily use for certain patients and indications. That does not mean daily use is right for everyone. Only your physician can determine which approach, if any, is appropriate for you.
A high-fat meal can slow the absorption of sildenafil, which may delay when its effect begins. Follow the instructions your physician gives you and the approved prescribing information for your product.
Absorption of tadalafil is not considered to be meaningfully affected by food according to approved prescribing information. Alcohol, however, can contribute to blood-pressure effects and dizziness with either medication.
Yes. Both medications support the body's own erectile response. Sexual stimulation is generally still required — neither one produces an erection on its own.
No. Neither medication directly increases testosterone. If low testosterone is a concern, that is a separate medical evaluation your physician can discuss with you.
No. PDE5 inhibitors are not aphrodisiacs and do not increase libido. They act on blood flow in response to sexual stimulation. Low desire has different causes and warrants its own conversation with a physician.
Switching is a medical decision. A physician can review your response, side effects, other medications and preferences and discuss whether a different approach may be appropriate and how any change should be handled.
The same applies in the other direction. Tell your physician what you have tried, how you used it and what happened — that information is what makes the next decision a good one.
Never on your own. Combining PDE5 inhibitors can affect the safety profile of treatment. In selected circumstances a physician may consider a customized combination approach when clinically appropriate; compounded formulations are not FDA-approved and are not appropriate for everyone.
Both are established prescription medications with important warnings, precautions, contraindications and drug interactions. Safety depends on your individual health, which is why a physician evaluation is required before treatment.
They are not appropriate for patients taking nitrates, and may be inappropriate with certain cardiovascular conditions, low or unstable blood pressure, some eye conditions, significant liver or kidney impairment, or certain interacting medications. Your physician reviews this with you.
No. PDE5 inhibitors can cause a dangerous drop in blood pressure when used with nitrate medications, including nitroglycerin, isosorbide and recreational nitrates known as poppers. Tell your physician if you take nitrates in any form, even occasionally.
Yes. Tadalafil and sildenafil are prescription medications in the United States and require evaluation by a licensed physician.
